Birth plan
I will be giving birth at QMH in a few weeks time and unlike with my first child I have not attended the antenatal classes at Tsan Yuk, where from memory they gave me a blank birth plan to fill in.
When I mentioned it to the Dr. today he dismissed it as 'unnecessary' but I felt a lot better having something in writing last time around.
Does anyone have a softcopy of a birth plan they have successfully used in the public system that they would be willing to let me use as a template?
Thanks in advance

I have one that a friend scanned for me for the same reason - if you PM me your email address I can just forward it to you...

I do have to say though - this time, I filled it all out but didn't take it out of my bag. It was helpful though for me to go through what I wanted different to last time - and I was sure to tell my husband the "important" things, and also to mention it to the nurses in the early stages of labour (eg, no episiotomy). They still followed my birth plan, but this time it was a more vocal one than one that they had written out there.
